Bow Well, Inc. respects the trust involved when someone shares personal information. We aim to collect only information reasonably needed to support our services and business operations, use it responsibly, and explain our practices in plain language.
This policy applies to the Bow Well website and information submitted directly to Bow Well. It does not replace the separate policies of Square, Google, CareCredit, Afterpay, Cash App, or other third-party services.
Information We Collect
We collect information you choose to provide when you complete the New Client Intake, call us, schedule an appointment, or otherwise communicate with Bow Well. This may include your name, date of birth, phone number, email address, mailing address, preferred contact method, service interests, wellness goals, accommodation requests, emergency contact information, acknowledgment, and typed signature.
Our website and hosting providers may also receive limited technical information, such as your browser type, device information, IP address, referring page, and basic activity or error logs needed to operate and secure the website.
How We Use Information
We may use information to respond to questions, prepare for requested services, contact you about an intake or appointment, provide client support, maintain business records, improve website operation, prevent misuse, protect our rights and safety, and comply with legal obligations.
Bow Well does not sell or rent personal information. We do not use intake information for targeted advertising.
Online Intake & Wellness Information
The online New Client Intake is limited to basic registration and service-planning information. More detailed physical health forms are completed upon arrival. Please do not use the website to send medical emergencies, diagnoses, test results, or other detailed medical records unless Bow Well specifically requests them through an appropriate process.
Bow Well provides nonmedical wellness services and education. This website is not an emergency service or a substitute for communication with a licensed healthcare professional.
When Information May Be Shared
We may share only the information reasonably necessary with service providers that support website hosting, secure data storage, communications, scheduling, payment processing, fraud prevention, or other business operations. These providers handle information under their own terms, privacy practices, and contractual obligations.
We may also disclose information when required by law, to protect clients or others, to investigate misuse, or as part of a lawful business reorganization or transfer. Bow Well and Bradley Chiropractic Center, LLC are separate companies. Information is not automatically shared between them merely because Bow Well operates inside the same location.
Square, Google & External Services
The Schedule Appointment buttons direct you to Bow Well’s Square booking page. Payments, appointment details, Apple Pay, Afterpay, CareCredit, and other activity completed through third-party services are governed by those providers’ privacy policies and terms.
The Contact page includes a Google Map and Google directions link. Google may receive device, browser, location, cookie, or usage information when you load or interact with those features. Bow Well does not control third-party privacy practices.
Security & Retention
Bow Well uses reasonable administrative, technical, and physical measures appropriate to the nature of the information we maintain. No website, transmission method, or storage system can be guaranteed completely secure.
We retain information only as long as reasonably needed for the purposes described in this policy, for client and business administration, and to satisfy applicable legal, accounting, or safety obligations. Information that is no longer needed may be securely deleted, anonymized, or disposed of.
Children & Minors
This website is not directed to children under 13, and children under 13 should not submit information through the website. A parent or legal guardian should contact Bow Well directly regarding any permitted service for a minor.
Bow Well’s service age rules remain separate from website privacy rules. Certain services may be available to ages 6–17 only with a parent or guardian and required consent.
Your Choices
You may call Bow Well to ask about personal information you previously provided, request a correction, update communication preferences, or request deletion when retention is not required for legitimate business, safety, or legal purposes. We may need to verify your identity before acting on a request.
You may choose not to submit an online intake and contact Bow Well about completing available registration steps in person. You may also adjust browser settings to limit cookies, although some third-party features may not function as intended.
